Well Child Visit Schedule
11 Years
What to expect at this visit:
- Measure and record your child's weight, height, blood pressure, and pulse in their growth chart, including their BMI.
- Provide immunizations.
- Review school performance and development.
- Discuss exercise and nutrition.
- Discuss the changes that occur during puberty and when they are likely to begin.

Tests/Screens/Assessments
- Preventive Screening Questionnaire (TB, Lead, Fluoride, Nutrition)
- Cholesterol Screening (performed once between 9-11 years, if not previously completed)
