Transferring to Our Practice

Your Child's Care Starts Here

Choosing a new provider for your child is an important decision, and we’re honored you’re considering our practice. We’re here to make the transition as simple and seamless as possible, and to ensure your child’s care continues without interruption.

  • Step 1: Register Your Child

    When you call to register your child(ren) we will:


    Verify insurance coverage

    We’ll confirm your child’s insurance information. You can also review our accepted plans and billing information here.


    Guide you through transferring medical records

    Our records coordinator will send you a New Patient Registration Packet, which includes a medical records release form. Once completed, you can email it to records@thepedcenter.com, and we will request your child’s records on your behalf. At a minimum, we need your child's immunization record and a copy of their most recent well visit before scheduling their first appointment.


    Review our vaccine policy

    Our providers follow the recommended AAP vaccination schedule and expect patients to stay up to date on all immunizations. If this approach does not align with your preferences, we respectfully recommend seeking care with a practice that is a better fit for your family. Our complete vaccine policy can be found here.

  • Step 2: Schedule Your Appointment

    Once our medical records coordinator receives your child’s records, we’ll contact you to schedule your first appointment.


    Choosing a provider

    We encourage you to review our providers’ bios so you can request a specific provider if desired. We recommend building a relationship with two to three providers for continuity of care.


    Flexible scheduling

    We understand that illnesses can be unpredictable, and your child may occasionally need to see a different provider. Our practice has several locations throughout Frederick County, and you may schedule your child’s appointment at whichever location is most convenient.

  • Step 3: Prepare for Your Appointment

    We use Phreesia, a secure mobile check-in system, to make your visit easier. You’ll receive a link by text or email before your appointment to begin the check-in process. Please plan to arrive 10–15 minutes early so we can ensure everything is ready for your visit.

  • Step 4: Concluding Your Visit

    Your provider will let you know if any follow-up appointments are necessary. Before you leave, please stop by the check-out desk so our team can assist you.


    After your visit, you’ll receive a short questionnaire by text or email. Your feedback helps us continue to improve and provide the best care possible. We would love to hear about your experience!
